  How do you support people to learn on a daily basis?

  To perform in a job we need to learn from we are doing well and what is not going well. Often we do not take the time to think about it. What do you do to help yourself and others learn. I found asking the question - what did you learn? - helpful, however maybe you have better ideas.
  
  One of the reasons that we can stuck is that we do not learn from our successes and mistakes. There must be some good ideas out there...
