At Helios Klinikum Duisburg we are offering you a full-time or part-time position as Case Manager (m/f/d) as soon as possible.

• Clarification and coordination of patient-specific care needs on admission, peristation and discharge

• Initiation of measures and monitoring
• Case-related control and management of the treatment process
• Advising, training and supporting the nursing and medical services in relation to the discharge management framework agreement and other legal requirements
• Participation in team meetings, ward rounds
• Reviewing and optimizing the quality of documentation
• Interface and communication management with all stakeholders and service providers involved

Completed training as a nurse (m/f/d) and experience in project management and/or management positions

Ideally an additional qualification, such as further training in case management, further specialist training or a Bachelor's degree in a nursing-related course of study

Willingness to take on responsibility and make independent decisions

Competence in developing, moderating and implementing solutions for complex issues

Professional and proactive communication skills, both in writing and by telephone, with all interfaces

Advanced computer skills, including experience with Microsoft Excel

Strong patient observation and analysis skills, strong interpersonal skills as well as independence and self-organization

Legally required proof of immunization against measles
