At the earliest possible date we are looking for you for our Helios Clinic in Müllheim as

• Professional and disciplinary management of the case management team
• Development, control and optimization of treatment processes and cross-sector care pathways
• Close cooperation with doctors, nursing staff and all professional groups involved to ensure optimal case management
• Responsibility for structured discharge management in close coordination with social services and external partners
• Analyzing and managing the course of treatment with regard to medical, nursing and economic aspects
• Process optimization with regard to patient admission, coding, length of stay and documentation

• Completed training as a nursing specialist (m/f/d)
• Sound professional experience in the field of case management or discharge management
• Ideally initial management experience
• Structured and solution-oriented way of working as well as high social and communication skills
• Organizational skills and a high degree of personal responsibility
