FLSA Status: Full-time, Salary Exempt
***Bonus*** $3,000 hiring bonus - $1000 after first 30 days of successful employment, $1000 after 90 days of employment, and $1000 after 6 months employmentPurpose:
This position involves providing direct support to individuals who have a history of homelessness and serious mental illness and require assistance in accessing housing in Ramsey County. This position will also support our apartment community with case management services. By collaborating with social service and housing providers, clients will be able to improve their housing stability, manage their health conditions more effectively, successful tenancy and enhance their overall quality of life.
Key Responsibilities:
Duties and Responsibilities:
· Complete individual assessments and plans for individuals to engage in housing search and housing sustainability. Regularly review assessments to ensure alignment with client goals for long-term housing stability.
· Assist clients in active housing search including but not limited to, touring housing options, completing housing applications, engaging with landlords and properties to create a person-centered relationship for the client. Assisting the client in seeking and arranging client move-in to housing.
· Ability to negotiate manageable and appropriate lease agreements with landlords.
· Monitor participant housing stability after move-in through home visits, recertifications, and communication with the landlord/property manager to resolve housing-related crises.
· Knowledgeable in accessing resources to assist and support the client in financial management, legal resources, life skills, employment services and supports, MH/CD services, connections to mainstream resources and tenant education.
· Provide case management services that address unique needs, preferences, and financial resources of clients and overcome barriers to housing access.
· Communicate and maintain relationships with all service providers involved in client care to ensure continuity of care and collaboration. Establish relationships with other housing agencies (i.e. HousingLink, Dakota and Ramsey CDA, etc.).
· Monitor risk factors that may create the potential for crisis. Implement crisis intervention strategies in a timely manner.
Required Skills/Abilities:
· Advanced verbal and written communication skills.
· Knowledge of major housing markets in Dakota County and Ramsey County.
· Ability to recognize the barriers individuals face who have mental illness, substance use, and histories of homelessness in finding housing.
· Ability to problem solve, including identify the essential factors in situations and actively engage individuals in solutions.
· Ability to self-motivate, manage multiple priorities, and adhere to deadlines.
· Exercising judgment, knowing when to use own discretion and when to seek supervision and guidance.?
Education and Experiences:
· Bachelor’s degree in social services or related field
· 4,000 hours experience in housing related services or homeless population.
· Minimum of one-year personal or working experience related to the homeless population.
· Minimum of one-year case management experience.
Additional Requirements:
· Must be ambulatory and able to negotiate stairways.
· A valid Driver’s License, reliable vehicle, satisfactory driving record, and auto insurance are required.
· Must not be actively receiving services from Guild and must not have received services within the last two years.
Working Conditions:
· This position is based at Guild St. Paul Area both at the Wabasha location and the Selby Apartments; however, a lot of work is in the field as services are provided to individuals wherever needed. This position requires transporting individuals in personal vehicle regularly. This position has the potential to come into contact with bodily fluids, communicable diseases, tobacco smoke, and/or household pets.

The salary range for this position is between $50,000-$57,000 annually. The range reflects the minimum and maximum salary for the position. Compensation will depend on the candidate’s professional experience, skills, and relevant education, training, and certifications.
