Helios Klinik Schleswig GmbH is looking for several coordinators for specialized outpatient palliative care (m/f/d) in full or part-time, permanent positions as soon as possible.

• Palliative counseling
• Coordination of SAPV admissions
• Initial visits
• Advising the cooperating care teams
• Further development and networking

• Completed training as a healthcare and nursing professional (m/f/d)
• Basic course in palliative care (160 hours) in accordance with SGB V§39a and willingness to undergo further and advanced training
• Professional experience in outpatient or inpatient palliative care
• Experience with crisis intervention and social counseling
• Independent and structured way of working as well as creativity, open-mindedness and resilience
• Sensitivity in dealing with patients and caregivers in the special situation of a hospital stay
• Legally required proof of immunization against measles
