We are looking for you as a part-time social worker (m/f/d) for the Clinic for Palliative Medicine as soon as possible.

• Collection and documentation of important socio-medical information
• Systemic counseling: family discussions to clarify responsibilities and needs, support for relatives
• Application and contact with cost bearers, e.g. for care degree, rehab/pension, hospice, appeal against rejected applications
• Information on legal aspects, e.g. legal care, advance care planning
• Organization of further care: Nursing home, hospice, care service, SAPV, volunteers, aids
• Guide function: arranging support for patients and relatives, e.g. self-help groups, bereavement services
• The aim is to provide optimal support for families, avoid "revolving door effects" and ensure prompt further care.

• University degree or extra-occupational further qualification in the field of social work
• Interest in interdisciplinary palliative care in a maximum care facility
• Willingness to integrate into the multi-professional team of the palliative care unit
• Professional skills, good communication skills and strong social skills in dealing with patients, colleagues and students
• Enthusiasm
• Creative ideas for the further development of patient care
• Legally required proof against measles
