• Planning and coordination of discharge management
• Networking with external providers of care, care and rehabilitation
• Assistance and advice for patients in personal and financial emergencies
• Organization of outpatient care and counseling of patients as well as referral of patients to
• Short-term care, retirement homes and hospices
• Initiation of rehabilitation as well as advice and application within the framework of social legislation
• Clarification of inpatient treatment costs for patients without health insurance

• Completed training as a registered nurse (m/f/d), health and paediatric nurse (m/f/d) or geriatric nurse (m/f/d) with several years of professional experience in the social sector or in various patient-related areas
• Good knowledge and experience in conducting conversations and advising patients and relatives
• A high degree of commitment, confident appearance, empathy, very good time management and pedagogical skills round off your profile
• Good IT skills and knowledge of social legislation SGB V and SGB XI desirable
• A high degree of flexibility, open-mindedness and commitment as well as the willingness to work together in an open, collegial and interdisciplinary manner
• Legally required proof of immunization against measles
