Fraser is hiring a Group Home Coordinator / Designated House Coordinator (DHC) to join our Community Living team. In this role, you will provide site-specific supervision and leadership to ensure high-quality care and continuous improvement in active treatment services at one of our supervised or supportive living locations. As the first point of leadership in the home, you will work closely with staff, clients, and families to ensure services meet Fraser’s standards and 245D requirements.

Responsibilities:

  • Provide on-site supervision and leadership to staff at the group home or residential location.

  • Ensure compliance with 245D regulations and Fraser policies.

  • Support active treatment service development and implementation.

  • Lead continuous quality improvement initiatives at the location.

  • Supervise staff and support their professional development.

  • Serve as a primary point of contact for clients, families, and community partners.

  • Monitor and maintain accurate client and staff records, including documentation required by regulations.

  • Participate in emergency response and preparedness activities as needed.

Requirements:

Candidates must qualify based in one of the following three ways, as assigned by 245D, which includes a combination of work experience and education.

  • HS Diploma or Equivalent AND four (4) years of full-time work experience (8,000 hours) providing direct care services to persons with disabilities or nursing/assisted living home experience
  • BA/BS in Human Services or related field AND 1 year of full-time work experience providing direct care services to persons with disabilities
  • Associate degree in Human Services or related field AND 2 years of full-time work experience (or equivalent) providing direct care services to persons with disabilities

Location, Schedule & Pay:

  • Location
    • Eden Prairie, MN
  • This position is full time Monday-Friday, standard business hours
  • The starting pay range for this role is $21.50 - $23.50 with an additional 2-dollar differential per hour depending on up qualifications and location
  • $1,000 Hiring Bonus for external applicants
