We are looking for a Social Pedagogue or Social Worker Discharge Management/Social Services (m/f/d) for our clinic in Bad Saarow with immediate effect .

• Coordination of post-inpatient measures: In close coordination with treating physicians and nursing staff
• Cooperation with external partners: rehabilitation and care facilities, authorities and institutions
• Patient transition: Professional organization of the transition to the outpatient or inpatient sector
• Social counseling: Advice for patients and relatives on social and socio-legal issues
• Support with social benefits: Accompanying patients and relatives in the utilization of support services and benefits

• Completed studies: (B.A.) in social pedagogy or social work
• Professional experience: Ideally several years of experience in clinical social work
• IT skills: Confident handling of MS Office and clinical information systems (HIS)
• Professional and social skills: High level of professionalism and empathy in dealing with patients and colleagues
• Interdisciplinarity: Thinking and acting in terms of networked, holistic cooperation
• Team spirit and motivation: Strong social skills, commitment and willingness to cooperate

The Helios Klinikum Bad Saarow is a hospital providing qualified standard care and, with 629 beds, is one of the largest and most modern clinics in the region. 19 specialist departments, three institutes, a central emergency department, a central laboratory and various certified centers provide a comprehensive range of state-of-the-art diagnostics and innovative therapies.

The Clinic for Anaesthesiology, Intensive Care Medicine and Pain Therapy and the Central Emergency Department/Rescue Medicine are structured according to the four pillars of the specialist field (AINS). The Intensive Care Medicine department consists of two interdisciplinary intensive care units with a total of 42 beds including a weaning unit. We are the Brandenburg ECMO center (vv and va-ECMO incl. pick-up service) and participate as a productive and successful study center in SepNet. Palliative care concepts are an integral part of our care. Both intensive care units are operated in shifts with an interdisciplinary medical presence under intensive care anesthesiology management. The clinic provides in-clinic emergency care and care for critically ill patients who come to the clinic via the Central Emergency Department and participates in the ambulance service.
