Position Summary
The Resource Navigator will provide assistance to patients/families finding reliable information about internal and external resources that can improve their health. The Resource Navigator will work inside Children's General Pediatrics Clinic to identify social conditions affecting families' health, connect them to resources as needed and follow-up after referrals have been made.
License/Certification/Registration required? No
Education:
*High School Graduate or GED Equivalent
Experience:
*Experience helping or serving others (Ex. customer service, interpreter, etc.)
*Experience working with diverse populations
*Experience using computer programs including Microsoft Office
Knowledge/Skills/Abilities:
*Native or fluent in more than one language (Spanish or Somali) - required
*Demonstrated cultural competency
*Strong interpersonal skills, particularly effective listening and ability to show empathy
*Comfortable with change and ability to be flexible, and be a proactive problem-solver
*Ability to build trust and connections in a clinic setting around sensitive issues for patients/families to ensure resource needs are identified and ultimately utilized
*Able to travel between clinics if needed
*Awareness of health care disparities, social determinants of health and grounding in health equity
*Strong organization skills, timeliness and attention to detail
*Computer knowledge and ability to effectively utilize technology
*Ability to multi task and prioritize
*Ability to work collaboratively and independently
*Strong written and verbal communication skills
The posted salary represents a market competitive range based on salary survey benchmark data for similar roles in the local or national market. When determining individual pay rates, we carefully consider a wide range of factors including but not limited to market indicators for the specific role, the skills, education, training, credentials and experience of the candidate, internal equity and organizational needs.
In addition to your salary, this position may be eligible for medical, dental, vision, retirement, and other fringe benefits. Positions that require night, weekend or on-call work may be eligible for shift differentials or premium pay.
