We build beautiful applications, but they break in unpredictable ways. Uncovering the real cause of such deficiencies can be time-consuming and often results in one-way roads. Such disasters are often attributed to human error. In all those breaches it was never a fault of a single person, and there were no processes and measures in place to prevent such things from happening.
